Merge QUE-298 branch (now carrying main's QUE-300 header-depth requirement)

ingest_fusion.py resolved to this branch's own depth handling, which
subsumes the incoming version: same warning-and-skip on a gridded table
without header depth, plus a tableFlag review record for the skip and
table-scope footnote markers reaching every line record.
